Zusammenfassung
Die mesenterische, inflammatorische veno-okklusive Erkrankung ist eine Krankheitsentität unbekannter Inzidenz und Ätiologie, bei der eine Entzündung und Nekrose mesenterialer Venen zu einem schweren ischämischen Schaden des betroffenen intestinalen Segments führen. Das klinische und endoskopische Erscheinungsbild kann vieldeutig sein. Eine moderne Mehrphasen-Computertomografie erlaubt jedoch unter Beachtung typischer Stigmata eine frühe Diagnosestellung. Da eine Operation des affektierten Segments den einzig kurativen Ansatz mit zudem minimaler Rezidivgefahr darstellt, kann eine Kenntnis des seltenen Krankheitsbilds dem Patienten einen riskanten Aufschub der Darmresektion und fehlerhafte Behandlungsansätze ersparen.
Abstract
Mesenteric, inflammatory veno-occlusive disease is an entity of unknown incidence and aetiology. Inflammation and necrosis of mesenteric veins leads to severe ischaemic damage of an intestinal segment. The clinical and endoscopic presentation can be ambiguous. However, modern multiphasic contrast-enhanced computed tomography allows early diagnosis, when characteristic features are considered. As local resection is the only known cure with minimal risk of recurrent disease, knowledge of this rare disease entity can spare our patients a risky delay of resection and erroneous therapeutic approaches.
